The contact information for doctors who treat Lyme (LLMDs), especially late-stage Lyme disease, is not readily available due to the highly political nature of the disease. Some states, like Massachusetts, have passed legislation protecting a doctors right to use long-term antibiotics. Many states however do not have such laws.
The Lyme Alliance of the Berkshires has lists available for the entire USA and Massacusetts on it's own.
